Effect of Microvascular Invasion Risk on Early Recurrence of Hepatocellular Carcinoma After Surgery and Radiofrequency Ablation

Summary: In this study, surgical resection (SR) was found to be associated with a lower rate of early recurrence compared to radiofrequency ablation (RFA) in patients with hepatocellular carcinoma (HCC) based on the risk of microvascular invasion (MVI). The predictive model developed in the study accurately predicted the risk of MVI in HCC patients with high accuracy, indicating that patients with MVI who underwent RFA were more prone to recurrence than those who underwent SR.

Summary: Microvascular invasion (MVI) is a critical risk factor for postoperative recurrence of hepatocellular carcinoma (HCC), and preoperative knowledge of MVI can aid in tailored surgical planning. This study found that deep learning (DL) is more effective in MVI prediction than traditional imaging methods, with gadoxetic acid-enhanced magnetic resonance imaging (EOB-MRI) proving to be superior to contrast-enhanced computed tomography (CE-CT) for MVI assessment.

Summary: This meta-analysis identified seven significant MRI features as predictors for microvascular invasion (MVI) in hepatocellular carcinoma (HCC), including larger tumor size, rim arterial enhancement, arterial peritumoral enhancement, peritumoral hypointensity on hepatobiliary phase imaging, nonsmooth margin, multifocality, and hypointensity on T1-weighted imaging. These MRI features can be useful in the management of HCC.
