The Devastating Clinical Consequences of Child Abuse and Neglect: Increased Disease Vulnerability and Poor Treatment Response in Mood Disorders

Elizabeth T. C. Lippard et al.

Summary: A large body of evidence has demonstrated the long-term consequences of childhood maltreatment, which increases the risk for psychiatric and medical disorders. This review summarizes the effects of childhood maltreatment on vulnerability to mood disorders, highlighting the increased prevalence and associated risks. It discusses the persistent alterations in the hypothalamic-pituitary-adrenal axis and inflammatory cytokines, as well as candidate genes, environmental factors, and neurobiological associations that may mediate these effects. The studies provide insights into modifiable mechanisms and guide treatment and prevention strategies.

Discontinuation of antidepressants after remission with antidepressant medication in major depressive disorder: a systematic review and meta-analysis

Masaki Kato et al.

Summary: Continuing treatment with antidepressants after achieving remission is recommended to reduce the risk of relapse of depressive symptoms. Different classes of antidepressants have varying effectiveness in preventing relapse, with tricyclics showing the highest effect size. Maintenance therapy for at least 6 months is crucial for relapse prevention.

Summary: This study found that guided iCBT was more effective than unguided iCBT for individuals with depression, particularly in those with moderate to severe depression. Unguided iCBT showed similar effectiveness among individuals with symptoms of mild/subthreshold depression. Personalized treatment selection is crucial to ensure optimal allocation of treatment resources for individuals with depression.

Establishing Evidence for Clinical Utility of a Neuroimaging Biomarker in Major Depressive Disorder: Prospective Testing and Implementation Challenges

Mary E. Kelley et al.

Summary: This study is the first prospective test of the clinical utility of using an imaging biomarker to select treatment for patients with major depressive disorder. Results show some evidence for the role of right anterior insula activity in the clinical choice of monotherapy, but the effect size is insufficient to use insula activity as a sole predictive biomarker of remission. The study also highlights the logistical difficulties in establishing clinical utility.

Summary: The study found a global upward trend in the incidence of cardiovascular diseases (CVDs) until 1996, followed by a downward trend. Nearly all countries experienced a significant decline in mortality rates from 1990 to 2017. Developed countries saw a faster reduction in incidence and mortality rates compared to developing countries, while the global MIR had a slight decrease with developing nations showing a more gradual decline.

Summary: This study identified 76 RCTs, with findings suggesting that behavioral activation may be beneficial and that relaxation may be harmful for treating depression. Baseline severity emerged as the strongest prognostic factor for endpoint depression, and combining human and automated encouragement reduced dropouts from treatment. Bias risk was low for randomization and missing outcome data, uncertain for deviation from intended interventions, and high for outcome measurement.
