Predictors of Functional Outcome After Thrombectomy in Patients With Prestroke Disability in Clinical Practice

Monica Millan et al.

Summary: This study aimed to evaluate the frequency of previously disabled patients treated with mechanical thrombectomy (MT), assess the safety and clinical response of these patients, and identify disabled patient characteristics associated with a better response to MT. The results showed that prestroke disability was not associated with a lower chance of achieving a favorable outcome at 90 days after MT. However, prestroke disability was independently associated with a higher risk of symptomatic intracranial hemorrhage and long-term mortality. Among prestroke disabled patients, those without diabetes and with favorable early ischemic signs had a better chance of achieving a favorable outcome after MT.

Economic Evaluation of Endovascular Treatment for Acute Ischemic Stroke

Lucie A. van den Berg et al.

Summary: This study aimed to evaluate whether endovascular therapy plus usual care is cost-effective compared to usual care alone in acute ischemic stroke patients. The results showed that endovascular treatment added to usual care is clinically effective and cost-saving in comparison to usual care alone for patients with acute ischemic stroke.

Impact of pre-stroke dependency on outcome after endovascular therapy in acute ischemic stroke

Lisa Oesch et al.

Summary: This study compared stroke patients with preexisting dependency to independent patients who underwent EVT. It found that the dependent patients were older, had more severe strokes, and more risk factors, but the preexisting dependency did not significantly affect clinical outcomes or mortality. Careful selection of care-dependent stroke patients for EVT is needed, as prior dependency does not greatly impact outcomes.

Prestroke Disability and Outcome After Thrombectomy for Emergent Anterior Circulation Large Vessel Occlusion Stroke

Adam de Havenon et al.

Summary: A post hoc analysis of stroke patients undergoing endovascular therapy revealed that those with premorbid disability were more likely to avoid accumulating further disability but had a higher chance of death compared to patients without disability.

Toward a more inclusive paradigm: thrombectomy for stroke patients with pre-existing disabilities

Robert W. Regenhardt et al.

Summary: The study found that while baseline disability was associated with absolute 90-day disability, there was no significant association with accumulated disability or other outcomes. Patients with baseline disability should not be routinely excluded from EVT based on baseline mRS alone.
