Hypophosphatemia after intravenous iron therapy: Comprehensive review of clinical findings and recommendations for management

Summary: Contemporary intravenous iron formulations allow for high doses of elemental iron to be administered, correcting total iron deficit in one or two infusions. Certain formulations can cause hypophosphatemia due to increased secretion of FGF23. Clinicians must recognize the clinical manifestations and monitor serum phosphate concentrations in patients receiving repeated doses of specific intravenous iron formulations to diagnose and manage hypophosphatemia effectively.

Summary: The study found that FCM has a higher incidence of hypophosphataemia compared to IIM, with a higher mean decrease in serum phosphate levels. Low baseline serum ferritin and transferrin saturation, as well as normal kidney function, were identified as significant predictors of hypophosphataemia.

FGF23-related hypophosphatemic rickets/osteomalacia: diagnosis and new treatment

Summary: FGF23 is a phosphaturic hormone produced by bone that reduces serum phosphate levels by suppressing phosphate reabsorption and absorption. Elevated FGF23 levels are associated with hypophosphatemic diseases, while patients with chronic hypophosphatemia from other causes have lower FGF23 levels. Various treatment options for diseases caused by excessive actions of FGF23 including TIO have limited effects and adverse events, with the novel therapy of FGF23 inhibition showing promise.
