Why does unilateral pulsatile tinnitus occur in patients with idiopathic intracranial hypertension?

Pengfei Zhao et al.

Summary: The study found that the prevalence of transverse sinus stenosis was significantly higher in IIH patients than in the controls, but it had no correlation with PT. The proportion of venous outflow laterality and the prevalence of sigmoid sinus diverticulum were significantly larger in IIH patients with PT.

Computational fluid dynamics study of the effect of transverse sinus stenosis on the blood flow pattern in the ipsilateral superior curve of the sigmoid sinus

Yanjing Han et al.

Summary: Blood flow impingement on the superior curve of the sigmoid sinus wall intensified with increasing degree of stenosis and decreased with increasing distance between the stenosis and the sigmoid sinus. The impact zone was generally confined to the anterior and lateral walls of the superior curve of the sigmoid sinus. When the stenosis was located far from the middle of the transverse sinus, the blood flow impingement on the sigmoid sinus wall was very weak.

Effects of different morphologic abnormalities on hemodynamics in patients with venous pulsatile tinnitus: A four-dimensional flow magnetic resonance imaging study

Xiaoshuai Li et al.

Summary: This study evaluated the effects of SSWD, TSS, and SSD on the hemodynamics of transverse-sigmoid sinus in venous PT patients, finding that TSS can increase blood velocity and form jet-like flow, while SSD is related to vortex formation but has no significant effect on blood velocity and flow.

The relationship between cerebral blood flow and venous sinus pressure: can hyperemia induce idiopathic intracranial hypertension?

Alexander Robert Bateman et al.

Summary: The relationship between cerebral venous sinus blood flow and pressure response is quadratic in nature, with the major cause being the degree of rotation induced in the flow. Elevated blood flow in children with IIH can explain the increased intracranial pressure found, secondary to the increase in venous pressure.

Novel grading system of sigmoid sinus dehiscence for radiologic evaluation of pulsatile tinnitus

Shelby Willis et al.

Summary: Sigmoid sinus dehiscence (SSD) is a common condition in non-PT patients, with 34% prevalence in this study. The study established a grading system for SSD, with 75% Grade 1 and 25% Grade 2 dehiscences. Grade 3 dehiscences were absent in this non-PT cohort.

Bone remodeling in sigmoid sinus diverticulum after stenting for transverse sinus stenosis in pulsatile tinnitus: A case report

Xiao-Yu Qiu et al.

Summary: Pulsatile tinnitus (PT) is a potentially disabling symptom that has received increasing attention. Multiple causes of PT have been confirmed by targeted treatment. Dynamic changes of related structures in PT patients with multiple causes after stenting for ipsilateral transverse sinus stenosis (TSS) have not been previously reported. PT associated with sigmoid sinus wall anomalies, TSS, and idiopathic intracranial hypertension can be cured by stenting for TSS, and bone remodeling around sigmoid sinus wall anomalies is a significant finding.

Multiphysics coupling study on the effect of blood flow pulsation in patients with pulsatile tinnitus

Zhenxia Mu et al.

Summary: This study investigated the effect of blood flow pulsation on patients with pulsatile tinnitus. The results showed that blood flow directly impacted the vessel wall in the sigmoid sinus wall dehiscence area, and changes in blood flow pulsation index affected pressure, displacement, and sound pressure level in the area. The study suggested that reducing blood flow pulsation could potentially alleviate the symptoms of pulsatile tinnitus in patients.

Multiphysics coupling numerical simulation of flow-diverting stents in the treatment of patients with pulsatile tinnitus

Zhenxia Mu et al.

Summary: Flow-diverting stents (FDS) showed potential effectiveness in treating patients with pulsatile tinnitus (PT) caused by sigmoid sinus diverticulum (SSD) and sigmoid sinus wall dehiscence (SSWD). By preventing blood flow impact on vessels and reducing short-term vessel vibration, FDS attenuated the degree of PT symptoms. Additionally, FDS led to thrombosis and long-term blockage in the SSWD area, showing promise as an effective treatment for PT.

CT venography correlate of transverse sinus stenosis and venous transstenotic pressure gradient in unilateral pulsatile tinnitus patients with sigmoid sinus wall anomalies

Pengfei Zhao et al.

Summary: The degree and length of ipsilesional TSS were positively correlated with TPG, while there was no statistical difference in TPG among TSSs of different shapes or locations. The degree, length of ipsilesional TSS, and contralateral transverse sinus dysplasia were significantly correlated with TPG.
