Successful Results of Intracytoplasmic Sperm Injection of a Chinese Patient With Multiple Morphological Abnormalities of Sperm Flagella Caused by a Novel Splicing Mutation in CFAP251

Summary: We report a case of a Chinese male patient from a nonconsanguineous family with multiple morphological abnormalities of sperm flagella (MMAF) caused by splicing mutations in CFAP251 (c.1192-3C>G), resulting in exon skipping and significantly decreased expression of CFAP251 protein. Interestingly, the patient's mother was a heterozygous carrier for the mutation, while the father was not, and the inheritance pattern was proven to be maternal uniparental disomy. Successful pregnancy was achieved through intracytoplasmic sperm injection.

This study using Cfap65 (-/-) mice showed that CFAP65 plays essential roles in sperm head shaping, acrosome biogenesis, and mitochondrial sheath assembly. Proteomic analysis revealed disruption of proteostatic system in CFAP65-associated sperm malformation.
