Pelvic Ring Injury Mortality: Are We Getting Better?

Lucas S. Marchand et al.

Summary: The study found a significant improvement in overall mortality rates of patients with pelvic ring injuries in recent years, with mortality rates decreasing from 9.1% to 4.9%. The improvement appears to coincide with changes in trauma resuscitation and the implementation of adjuvant treatments for managing patients with severe hemorrhagic shock.

The past, present, and future management of hemodynamic instability in patients with unstable pelvic ring injuries

Joshua A. Parry et al.

Summary: Pelvic ring injuries with hemorrhagic shock historically had a mortality rate greater than 30%. Through a multidisciplinary protocol prioritizing preperitoneal pelvic packing over angiography, deaths from hemorrhage have been reduced by 30%. However, many trauma centers do not routinely stabilize pelvic ring injuries or control pelvic hemorrhage, leading to high mortality rates.

Summary: This study retrospectively reviewed all patients managed with REBOA from the ABOTrauma Registry and the AORTA database internationally. It found that Delta SBP predicts mortality in patients treated with REBOA and can serve as a surrogate marker for hemorrhage status.

Resuscitative Endovascular Balloon Occlusion of the Aorta vs Pre-Peritoneal Packing in Patients with Pelvic Fracture

Samer Asmar et al.

Summary: A study comparing the outcomes of REBOA and/or PP as temporizing measures in blunt pelvic fracture patients found that REBOA was associated with improved outcomes, including lower mortality rates and faster times to intervention. Further clinical trials are needed to determine the optimal use of REBOA in these patients.
