Left Ventricular Mechanics Differ in Subtypes of Aortic Stenosis Following Transcatheter Aortic Valve Replacement

Summary: By analyzing patients with different subtypes of severe AS after TAVR, it was found that myocardial mechanics improved post-TAVR and the recovery of GLS was associated with a survival benefit. Patients with low-flow AS showed significant improvement in GLS post-TAVR, but still had impaired function.

The prognostic impact of low-gradient AS is heterogeneous, and treatment should be based on individual patient risk and comorbidities.

Summary: Calcific aortic valve disease is associated with increased stiffness and reduced motion of AV leaflets, which may progress into aortic stenosis. This study demonstrated that two-dimensional speckle-tracking echocardiography could potentially be used for non-invasive evaluation of AV biomechanical properties. Deformation of AV leaflets measured through STE showed significant differences among patients with different degrees of AV disease and correlated with transvalvular hemodynamic parameters and AV calcification.
