Arrhythmic risk stratification by cardiac magnetic resonance tissue characterization: disclosing the arrhythmic substrate within the heart muscle

Summary: Sudden cardiac death is a significant global health issue, and identifying individuals at high risk is crucial for selecting candidates for implantable cardioverter defibrillator therapy. Current strategies for arrhythmic stratification rely on echocardiography-measured left ventricular ejection fraction and New York Heart Association functional status, but have suboptimal accuracy. Cardiac magnetic resonance provides a more comprehensive evaluation, identifying cardiac fibrosis as a potential arrhythmic substrate, and late gadolinium enhancement has emerged as a strong prognostic marker, adding value in ischemic heart disease and non-ischemic cardiomyopathies.

Prognostic significance of unexplained left ventricular hypertrophy in patients undergoing carpal tunnel surgery

Summary: Carpal tunnel syndrome is linked to increased cardiovascular morbidity and can be a red-flag for cardiac amyloidosis. A study found that over 30% of patients with carpal tunnel syndrome had left ventricular hypertrophy, with a significant portion showing echocardiographic features of underdiagnosed cardiac amyloidosis. Patients with unexplained left ventricular hypertrophy were associated with higher mortality and heart failure development rates compared to others.

Viral presence-guided immunomodulation in lymphocytic myocarditis: an update

Summary: Recent statements from European and American societies recommend ruling out viral presence in endomyocardial biopsy through PCR analysis before initiating immunosuppression or immunomodulation in acute lymphocytic myocarditis with life-threatening scenarios. However, there is currently a lack of definitive evidence on the survival benefits of immunomodulation guided by viral presence, and distinguishing causative agents from innocent bystanders among detected viruses remains challenging.

Myocarditis and inflammatory cardiomyopathy: current evidence and future directions

Summary: Inflammatory cardiomyopathy is a disease characterized by inflammatory cell infiltration into the myocardium and is predominantly mediated by viral infection. Despite extensive research, inflammatory cardiomyopathy complicated by left ventricular dysfunction, heart failure or arrhythmia is associated with a poor prognosis. The relative roles of the pathogen, host genomics and environmental factors in disease progression and healing are still under discussion.
