Protease-Activated Receptor Antagonist for Reducing Cardiovascular Events-A Review on Vorapaxar

Summary: Acute Coronary Syndrome (ACS) includes unstable angina, non-ST elevation myocardial infarction, and ST elevation myocardial infarction, which require urgent management to prevent future morbidity and mortality. Dual antiplatelet therapy, specifically aspirin and clopidogrel, is currently recommended for ACS management. However, Vorapaxar, a novel antiplatelet therapy, is being researched for improved outcomes in ACS patients. It has been approved for reducing thrombotic cardiovascular events in patients with a history of myocardial infarction or peripheral arterial disease, but with increased bleeding risk. Further subgroup analysis suggests potential benefits of Vorapaxar in peripheral artery disease, coronary artery bypass grafting, and ischemic stroke, but more research and trials are needed to determine its utility.

Since then, alteplase has become the standard of care in clinical settings, with improvements in quality measures such as door-to-needle times and specialized stroke unit care. The past decade has seen an expansion in the use of alteplase in prehospital settings, offering faster thrombolytic therapy through mobile stroke units and telestroke services.
