Controversies about the systematic preoperative pharmacological treatment before pheochromocytoma or paraganglioma surgery

Pheochromocytoma surgery without systematic preoperative pharmacological preparation: insights from a referral tertiary center experience

Summary: Despite significant advances in imaging, genetics, surgical, and anesthetic innovations, morbidity in pheochromocytoma surgery remains significant. This study aimed to identify predictive factors of global and cardiovascular morbidity following unilateral laparoscopic adrenalectomy for pheochromocytoma. The results showed that surgery without systematic pharmacological preparation and even without preoperative antihypertensives can be safe and feasible for selected patients.

Influence of secretory phenotype and preoperative preparation on surgical outcome in pheochromocytoma

Summary: The occurrence of haemodynamic complications during surgery was not significantly affected by the secretory phenotype, but noradrenergic tumors show a worse post-surgical outcome. Adequate preoperative alpha-blockade is beneficial in reducing hemodynamic complications for pheochromocytoma patients.
