Comorbidities Drive the Majority of Overall Mortality in Low-Risk Mucinous Pancreatic Cysts Under Surveillance

Summary: This study examined the comorbidity burden among low-risk pancreatic cyst patients and found that baseline age-adjusted CACI had good predictive capacity for 4-year extrapancreatic mortalities. Patients with a higher CACI were associated with significantly higher overall mortalities compared to patients with a lower CACI.

Summary: In patients with BD-IPMN who show no significant changes in the first five years, the incidence of developing pancreatic cancer remains low at 1.0%, while the incidence of developing HRS/WF/WEUS is higher at 29.0%. PC-related mortality is higher in the HRS/WF/WEUS group. These risks should be considered in conjunction with overall patient mortality when making decisions about surveillance beyond the initial five-year period.

Natural history and optimal treatment strategy of intraductal papillary mucinous neoplasm of the pancreas: Analysis using a nomogram and Markov decision model

Summary: This study investigated the natural history of IPMN and proposed optimal treatment strategies based on malignancy risk prediction using a nomogram and decision model. Main duct and mixed-type IPMN have a high risk of malignancy (79.9%) and surgery is recommended, while branch duct IPMN is very indolent (5.9%) and surveillance may be appropriate. Decision-making regarding surgery or surveillance should consider patients' health status, malignancy risk, and institutional experience.
