Guidance for Timely and Appropriate Referral of Patients With Advanced Heart Failure: A Scientific Statement From the American Heart Association

Summary: Among the estimated 6.2 million Americans living with heart failure, approximately 5% per year may progress to advanced, or stage D, disease. Advanced heart failure has a high morbidity and mortality, emphasizing the importance of early recognition and referral to advanced HF centers. Delayed or lack of referral can have important adverse consequences for patients who may benefit from advanced HF evaluation.
