COVID-19 and myocarditis: a systematic review and overview of current challenges

Myocarditis and inflammatory cardiomyopathy: current evidence and future directions

Summary: Inflammatory cardiomyopathy is a disease characterized by inflammatory cell infiltration into the myocardium and is predominantly mediated by viral infection. Despite extensive research, inflammatory cardiomyopathy complicated by left ventricular dysfunction, heart failure or arrhythmia is associated with a poor prognosis. The relative roles of the pathogen, host genomics and environmental factors in disease progression and healing are still under discussion.

Microthrombi as a Major Cause of Cardiac Injury in COVID-19 A Pathologic Study

Summary: Analysis of heart tissue from 40 deceased COVID-19 patients revealed that microthrombi were the most common pathological cause of myocyte necrosis. Female patients with chronic kidney disease and shorter symptom onset to admission time were more likely to have myocyte necrosis. Tailored antithrombotic strategies may be useful to counteract the cardiac effects of COVID-19 infection.

Myocardial Injury in Severe COVID-19 Compared With Non-COVID-19 Acute Respiratory Distress Syndrome

Summary: The study found that over 50% of intubated patients with severe COVID-19 had myocardial injury, which was associated with a higher mortality rate. Myocardial injury in COVID-19 was closely related to baseline comorbidities, age, and multisystem organ dysfunction. Compared with ARDS patients without COVID-19, COVID-19 patients were older and had poorer cardiac function, but had a lower incidence of myocardial injury.

Summary: COVID-19 is associated with cardiac injury and dysfunction, with cardiac infection by SARS-CoV-2 being common among patients who have died from the disease but often with only rare infected cells. Cardiac infection by SARS-CoV-2 is linked to more cardiac inflammation and electrocardiographic changes. Nonbiologic immunosuppression is associated with lower incidences of myocarditis and cardiac infection by SARS-CoV-2.

There was variability in prevalence across universities, and testing protocols were closely associated with the detection of myocarditis. The unique CMR imaging data highlight the need for standardized timing and interpretation of cardiac testing for safe return to play in athletes.
