Despite lack of definitive evidence, mechanical circulatory support devices are frequently utilized in managing cardiogenic shock.

Summary: This study found that in patients with acute myocardial infarction complicated by cardiogenic shock, a high Vasoactive Inotropic Score was associated with higher in-hospital and follow-up mortality. The predictive value of Vasoactive Inotropic Score for in-hospital mortality was significantly better in acute myocardial infarction patients with cardiogenic shock receiving medical treatment alone compared to those treated with mechanical circulatory support like intra-aortic balloon pump or extracorporeal membrane oxygenation.

Impella versus extracorporal life support in cardiogenic shock: a propensity score adjusted analysis

Summary: In patients with cardiogenic shock, the adjusted mortality rates were similar between Impella and ECLS. Baseline lactate level was identified as an independent predictor of mortality. The type of device used may be less important compared to other factors in patients with profound cardiogenic shock.

Summary: The ECLS-SHOCK trial is a 420-patient controlled, international, multicenter, randomized, open-label trial aiming to investigate the efficacy and safety of ECLS in addition to early revascularization in acute myocardial infarction complicated by cardiogenic shock. Patients will be randomized to either ECLS plus early revascularization or no-ECLS treatment arms, with 30-day mortality as the primary efficacy endpoint.

Summary: The study found that the use of Impella for treating AMICS increased annually from 2012 to 2017, reaching 19.9% in 2017. Compared to IABP, Impella use was associated with higher in-hospital mortality and transfusions, but not with acute kidney injury or stroke. Impella use was also linked to higher hospital costs.
