Distinguishing immune activation and inflammatory signatures of multisystem inflammatory syndrome in children (MIS-C) versus hemophagocytic lymphohistiocytosis (HLH)

Summary: This study compares the inflammatory profile differences between patients with MIS-C, coronavirus disease 2019, and HLH. The study found that both MIS-C and HLH patients showed T-cell activation and increased proinflammatory cytokines, but the levels were higher in HLH patients. Unique inflammatory features of MIS-C included elevation in T(H)2 inflammatory cytokines and cytokine mediators of angiogenesis and tissue repair.

Summary: Hemophagocytic lymphohistiocytosis (HLH) is characterized by acute T-cell activation, with CD38(high)/HLA-DR+ effector cells being most pronounced. In contrast, sepsis does not show this distinctive T-cell activation. CD38(high)/HLA-DR+ cells concentration above 7% has strong predictive value for distinguishing HLH from early sepsis or healthy controls.

Summary: This study found that untreated sHLH/MAS patients had higher levels of phosphorylated STAT1 in monocytes compared to glucocorticoid-treated patients, indicating a hyperreactivity to IFN gamma stimulation. The T cell repertoire returns to baseline soon after MIS-C resolution.
