Invasion of a Recurrent Laryngeal Nerve from Small Well-Differentiated Papillary Thyroid Cancers: Patient Selection Implications for Active Surveillance

Samantha K. Newman et al.

Summary: The success of managing low-risk papillary thyroid cancer (PTC) through active surveillance heavily relies on proper patient selection, particularly with regards to tumor location and size. A study conducted on patients at Memorial Sloan Kettering Cancer Center revealed that gross invasion of the recurrent laryngeal nerve (RLN) by tumors ≤ 2 cm is rare. Proper patient selection should consider not only the size and growth rate of the tumor, but also its location in relation to RLNs.

Thyroid lobectomy as a cost-effective approach in low-risk papillary thyroid cancer versus active surveillance

Mohanad R. Youssef et al.

Summary: In middle-aged patients with low-risk papillary thyroid cancer, lobectomy is a cost-effective strategy while active surveillance becomes cost-effective starting at the age of 69. Understanding these nuances can help personalize long-term management for patients.

Indications and Strategy for Active Surveillance of Adult Low-Risk Papillary Thyroid Microcarcinoma: Consensus Statements from the Japan Association of Endocrine Surgery Task Force on Management for Papillary Thyroid Microcarcinoma

Iwao Sugitani et al.

Summary: The management of patients with low-risk papillary thyroid microcarcinoma (PTMC; T1aN0M0) has become an important clinical issue, with the adoption of active surveillance (AS) to avoid overtreatment gradually increasing worldwide. However, not all PTMCs are suitable for AS, and the Japan Association of Endocrine Surgery has produced consensus statements regarding indications and strategies for AS in adult patients diagnosed with low-risk PTMC. These statements provide guidelines for AS and recommend immediate surgery for certain high-risk features in PTMC patients.

Interobserver Reproducibility in Sonographic Measurement of Diameter and Volume of Papillary Thyroid Microcarcinoma

Sae Rom Chung et al.

Summary: The study aimed to evaluate the reproducibility of the measurement of maximum diameter and volume of papillary thyroid microcarcinoma on ultrasound. The findings indicated that a difference of up to 24% in maximum diameter and 72% in volume may be considered to be within measurement error, providing valuable information for active surveillance.

Presence of TERT ± BRAF V600E mutation is not a risk factor for the clinical management of patients with papillary thyroid microcarcinoma

Jeonghun Lee et al.

Summary: The study showed that TERT +/- BRAF V600E mutations in patients with papillary thyroid microcarcinomas are not associated with aggressive clinical, radiological, or pathological features, and the presence of lymph node metastasis is not influenced by mutation status.

Tumor Volume Doubling Time in Active Surveillance of Papillary Thyroid Microcarcinoma: A Multicenter Cohort Study in Korea

Summary: The study found that TVDT <5 years is a crucial predictor of disease progression in PTMC during active surveillance, indicating new LN metastasis development and tumor enlargement. This early determination of TVDT could assist in tailoring follow-up intensity and determining the need for early surgical intervention.

Ultrasound-Guided Radiofrequency Ablation Versus Thyroid Lobectomy for Low-Risk Papillary Thyroid Microcarcinoma: A Propensity-Matched Cohort Study of 884 Patients

Lin Yan et al.

Summary: This study compared the clinical outcomes of radiofrequency ablation (RFA) versus thyroid lobectomy (TL) for low-risk papillary thyroid microcarcinoma (PTMC) and found comparable results in a four-year follow-up period. RFA, as a minimally invasive modality, may be a promising alternative treatment option for low-risk PTMC patients.

Long-Term Efficacy of Ultrasound-Guided Laser Ablation for Papillary Thyroid Microcarcinoma: Results of a 10-Year Retrospective Study

Ho Jin Kim et al.

Summary: This study evaluated the 10-year efficacy and safety of laser ablation (LA) for the treatment of solitary papillary thyroid microcarcinoma (PTMC). The results showed that LA was well tolerated by patients, with a disappearance rate of 100% in ablation areas after 12 months. Thyroid hormone and autoantibody levels did not significantly change post-treatment, and the majority of patients did not experience recurrence. For those with recurrence, surgery was performed successfully.

Multifocality and Progression of Papillary Thyroid Microcarcinoma During Active Surveillance

Ryuta Nagaoka et al.

Summary: In patients undergoing AS, multifocality in PTMC has not been proven to be a significant risk factor for disease progression, and there was no significant difference in the 10-year progression rate compared to unifocal disease. Many patients can avoid total thyroidectomy and subsequent surgical complications.

Diagnostic Strategies for Thyroid Nodules Based on Ultrasonographic Findings in Japan

Hiroki Shimura et al.

Summary: This article discusses the increasing incidence of thyroid cancer globally, mainly attributed to the widespread use of imaging examination methods. Ultrasonography plays a crucial role in evaluating thyroid nodules and diagnosing benign or malignant nodules. The diagnostic criteria and management guidelines in Japan are compared to those in other countries, highlighting the importance of sonographic findings in assessing the risk of thyroid cancer.
