Influence of Monitoring Strategy on Assessment of Ablation Success and Postablation Atrial Fibrillation Burden Assessment: Implications for Practice and Clinical Trial Design

Martin Aguilar et al.

Summary: This study aimed to identify a noninvasive rhythm monitoring strategy that accurately detects arrhythmia recurrences and approximates the atrial fibrillation (AF) burden derived from continuous monitoring. The results showed that most noninvasive monitoring strategies had poor sensitivity for detecting arrhythmia recurrence, while continuous long-term monitoring closely approximated the gold standard implantable cardiac monitor (ICM). Estimated AF burden from short-duration monitors significantly overestimated the true burden in patients with recurrences. Increasing monitoring duration improved the correlation and concordance between noninvasive estimates and the invasive AF burden.

Cryoablation or Drug Therapy for Initial Treatment of Atrial Fibrillation

Summary: In patients with symptomatic, paroxysmal atrial fibrillation, catheter cryoballoon ablation was associated with a significantly lower rate of atrial fibrillation recurrence compared to antiarrhythmic drug therapy. The ablation group also had better outcomes in terms of atrial tachyarrhythmia occurrence and atrial fibrillation burden.

Summary: The study aimed to evaluate the impact of using a 2-part AI filter on the PPV of ILR-detected AF events. Results showed a significant increase in PPV after employing the DNN filter, with the greatest improvement seen in AF episodes lasting over 30 minutes.

Summary: Atrial fibrillation ablation has been shown to reduce AF burden and improve quality of life, but the appropriate threshold of AF burden as an outcome measure is still unclear. Further research is needed to address this question.
