Comparative Efficacy of Root Repair versus Partial Meniscectomy and Observation for Patients with Meniscus Root Tears

Wenli Dai et al.

Summary: This study compared the efficacy of root repair versus partial meniscectomy and observation in patients with meniscus root tears. The results showed that root repair was associated with lower postoperative Kellgren-Lawrence grade and progression to arthroplasty rate compared to partial meniscectomy. Additionally, root repair was associated with lower arthroplasty rate and better International Knee Documentation Committee score compared to observation. The use of pullout technique for root repair was also found to be beneficial in terms of reducing OA progression and improving function scores. Overall, current evidence suggests that root repair with pullout technique may be a better choice for patients with isolated meniscus root tears in terms of OA progression, progression to arthroplasty, and functional outcomes.

Preoperative Meniscal Extrusion Predicts Unsatisfactory Clinical Outcomes and Progression of Osteoarthritis after Isolated Partial Medial Meniscectomy: A 5-Year Follow-Up Study

Joao V. Novaretti et al.

Summary: The study aimed to investigate the association between preoperative meniscal extrusion and clinical outcomes and progression of osteoarthritis, and to determine the extent of meniscal extrusion associated with unsatisfactory clinical outcomes and progression of osteoarthritis. The findings showed that meniscal extrusion greater than 2.2 mm was associated with unsatisfactory clinical outcomes and progression of osteoarthritis. Higher BMI and horizontal and root tears were associated with greater preoperative meniscal extrusion.

Steep posterior slope and shallow concave shape of the medial tibial plateau are risk factors for medial meniscus posterior root tears

Summary: The study found that patients with medial meniscus posterior root tears had a steep medial tibial slope and shallow medial tibial plateau concavity. Body mass index, medial tibial slope, and medial tibial plateau depth were significantly associated with medial meniscus posterior root tears.

Concurrent arthroscopic meniscal repair during open-wedge high tibial osteotomy is not clinically beneficial for medial meniscus posterior root tears

Xiurong Ke et al.

Summary: Concurrent arthroscopic meniscal repair during open-wedge high tibial osteotomies (OWHTOs) can increase the healing rate of medial meniscus posterior root tears (MMPRTs), but it does not lead to significant clinical benefits in terms of clinical outcomes.

Placement of an anatomic tibial tunnel significantly improves the medial meniscus posterior extrusion at 90° of knee flexion following medial meniscus posterior root pullout repair

Yusuke Kamatsuki et al.

Summary: This study found that the closer the tibial tunnel position to the anatomic attachment of the MM posterior root, the more effective the reduction in MMPE at 90 degrees knee flexion after pullout repair. An anatomic tibial tunnel significantly improves the postoperative MMPE at 90 degrees knee flexion.

Summary: This study aimed to investigate the impact of medial meniscus posterior root tear (MMPRT) on medial open-wedge high tibial osteotomy (MOWHTO). Summary: In patients with MMPRT, TSS repair results in a significant decrease in MMME and IMSI compared to F-MMA repair, suggesting that TSS repair is more effective in restoring loading stress to the posterior horn of the medial meniscus.

Preoperative varus alignment and postoperative meniscus extrusion are the main long-term predictive factors of clinical failure of meniscal root repair

Kyu Sung Chung et al.

Summary: This study identified preoperative varus alignment and increased meniscal extrusion after surgery as predictors of clinical failure after medial meniscus posterior root tears (MMPRTs) repair in the long-term. These negative prognostic factors should be considered when performing root repair in MMPRTs.

Summary: Transtibial pullout repair for medial meniscus posterior root tears in patients with early osteoarthritis of the knee improved clinical outcomes and showed a negative correlation between high meniscus healing scores and ICRS grades in the medial compartment loading area. This study suggests early surgery should be considered for patients with mild osteoarthritic knees who develop MMPRTs.
