Neutrophil-to-lymphocyte ratio in bronchoalveolar lavage from IPF patients: a novel prognostic biomarker?

Miriana D'alessandro et al.

Summary: This study demonstrates for the first time that the bronchoalveolar lavage (BAL) neutrophil-to-lymphocyte ratio (NL-ratio) is higher in idiopathic pulmonary fibrosis (IPF) patients compared to sarcoidosis and chronic hypersensitivity pneumonitis (cHP) patients. The NL-ratio is significantly correlated with lung function parameters and may be helpful in optimizing the management and prognosis of IPF patients.

A proposed role of neutrophil extracellular traps and their interplay with fibroblasts in ANCA-associated vasculitis lung fibrosis

Miguel Negreros et al.

Summary: ANCA-associated vasculitides (AAV) include three diseases: granulomatosis with polyangiitis, microscopic polyangiitis (MPA), and eosinophilic granulomatosis with polyangiitis. These diseases are characterized by small vessel inflammation and multiorgan involvement, with lung fibrosis being a common complication. The release of neutrophil extracellular traps (NETs) is associated with fibrosis development, but the exact mechanisms are not fully understood. This review discusses important proteins in NETs and proposes mechanisms for their impact on fibrotic phenotypes leading to lung fibrosis.

Systematic Review and Metanalysis of Oncomarkers in IPF Patients and Serial Changes of Oncomarkers in a Prospective Italian Real-Life Case Series

Miriana D'Alessandro et al.

Summary: This study focuses on identifying multiple biomarker signatures, such as combinations of oncomarkers, that are widely and routinely available in biochemistry laboratories. The combination of clinical parameters and biological markers could help achieve more accurate results regarding prognosis and response to treatment in IPF. The results could pave the way for a more personalized medical approach to patients affected by IPF.

Serum Concentrations of KL-6 in Patients with IPF and Lung Cancer and Serial Measurements of KL-6 in IPF Patients Treated with Antifibrotic Therapy

Miriana D'Alessandro et al.

Summary: Serial measurements of serum KL-6 in IPF patients treated with Nintedanib for 24 months showed higher levels in IPF-LC patients compared to other groups, suggesting a potential biomarker for poor prognosis. Many IPF patients on Nintedanib experienced stabilization of lung function and KL-6 concentrations over the study period.

Summary: The study found that increased NET formation is significantly associated with sepsis-induced DIC incidence and mortality in sepsis patients, with ELA2 and H3 being able to predict DIC occurrence effectively. Furthermore, ELA2 and H3 showed a significant correlation with endothelial cells and autophagy levels.

Summary: NFIL3 is highly expressed in gout patients and promotes neutrophil autophagy and NET formation by enhancing REDD1 transcription; REDD1 enhances neutrophil autophagy and NET formation by inhibiting the mTOR pathway, and silencing NFIL3 reduces inflammatory injury in acute gouty arthritis mice.

Neutrophil Extracellular Traps Induce the Epithelial-Mesenchymal Transition: Implications in Post-COVID-19 Fibrosis

Laura Pandolfi et al.

Summary: This study found that NETs play a role in inducing epithelial-mesenchymal transition of lung epithelial cells after severe SARS-CoV2 infection. Analysis of samples from severe COVID-19 patients showed high concentrations of NETs correlated with neutrophil count, indicating their potential involvement in disease progression. In vitro models revealed that induction of NETosis by SARS-CoV2 and secretion of AM factors are necessary to trigger a complete EMT expression pattern, shedding light on a possible mechanism for lung fibrosis post-SARS-CoV2 infection.
