Expression of CYP11B1 and CYP11B2 in adrenal adenoma correlates with clinical characteristics of primary aldosteronism

Chang Ho Ahn et al.

Summary: Variable expression of steroidogenic enzymes in adrenal adenoma underlies the clinical heterogeneity of PA and is associated with treatment outcomes. CYP11B1 expression was positively correlated with the severity of hypercortisolemia, and CYP11B2 was positively correlated with that of hyperaldosteronism. Patients with absent clinical improvement after adrenalectomy had lower CYP11B2 expression than those with complete success.

Evaluation of Intra-Renal Stiffness in Patients with Primary Aldosteronism

Luigi Petramala et al.

Summary: Aldosterone plays an important role in cardiovascular, metabolic, and renal damage in hypertensive patients. This study aimed to evaluate parameters obtained through eco-color Doppler study in asymptomatic patients with Essential Hypertension (EH) and Primary Aldosteronism (PA). The results showed that PA patients had higher renal filtration rate and albumin excretion compared to EH patients, as well as differences in various indices measured by eco-color Doppler.

Screening for primary aldosteronism in the hypertensive obstructive sleep apnea population is cost-saving

Summary: Rigorous screening for primary aldosteronism in patients with hypertension and obstructive sleep apnea is cost-saving due to cardiovascular risk reduction. Cost should not be a barrier to improving adherence to primary aldosteronism screening.

Outcomes in unilateral primary aldosteronism after surgical or medical therapy

Troy H. Puar et al.

Summary: The outcomes of surgery and medication treatments were comparable in patients with unilateral primary aldosteronism. While medical therapy may offer similar cardiovascular protection, surgery resulted in better systolic blood pressure control and lower dosages of antihypertensive medications, making it the recommended option for unilateral PA patients who can tolerate it.

Minimally invasive partial versus total adrenalectomy for unilateral primary hyperaldosteronism-a retrospective, multicenter matched-pair analysis using the new international consensus on outcome measures

Franck Billmann et al.

Summary: This study evaluated the clinical and biochemical efficacy of partial minimally invasive adrenalectomy versus total minimally invasive adrenalectomy for treating unilateral primary hyperaldosteronism. The results showed that both procedures had comparable success rates, but partial minimally invasive adrenalectomy may reduce the incidence of postoperative hypocortisolism and hypoglycemia, indicating it as a good surgical option.

Modulating ALDH2 reveals a differential dependence on ROS for hypertrophy and SR Ca2+ release in aldosterone-treated cardiac myocytes

Maria Guadalupe Montiel-Jaen et al.

Summary: The study found that high aldosterone levels are associated with atrial fibrillation and other heart diseases. Culturing adult rat atrial myocytes with aldosterone led to changes in cell size, Ca2+ homeostasis, ROS, and NO levels, as well as alterations in the expression of certain proteins. NOS and ALDH2 may play a role in preventing adverse effects of aldosterone.

Summary: The new non-steroidal MRAs may have at least equivalent efficacy to steroidal MRAs but potentially offer a better safety profile in patients with heart failure and/or kidney disease. Further research is needed to understand the molecular differences in their mode of action and identify predictive biomarkers for MRA response.

Summary: The risk of new-onset atrial fibrillation remains elevated up to 3 years in patients with primary aldosteronism compared with essential hypertension, especially peaking at 1 year, and gradually declining afterwards to levels comparable with essential hypertension.
