Geographic distribution of Pythium insidiosum infections in the United States

Don Nguyen et al.

Summary: This study describes the geographic distribution of Pythium insidiosum infections in dogs, horses, and other animal species in the US. Most infections were found in horses and dogs in the southeastern US, with no cases found west of longitude 100 degrees W in Texas. Few cases were diagnosed in west-coast states. Equine cases were more common in the summer and fall months, while canine cases were more common between September and February. Cases in other species were discovered in the same geographic areas as dogs and horses.

Summary: Pythiosis in equidae in northeastern Brazil is endemic, with a higher incidence after the rainy season. The disease presents as a chronic condition, characterized by nodules or tumor-like masses on the limbs and ventral thoracoabdominal wall. Detection of intralesional kunkers is a key for presumptive diagnosis, with confirmation through histopathology, immunohistochemistry, culture-based or molecular methods recommended.

Summary: The study reconstructed the distribution of Pythium insidiosum in the Chincoteague National Wildlife Refuge in Virginia, finding the pathogen throughout the area with significant monthly variation in suitability, primarily influenced by temperature and land-surface water. The suitability map could aid in management decisions, such as monitoring horses for lesions during high risk months.

Summary: PIA immunotherapy has shown effectiveness in reducing morbidity and mortality of pythiosis by shifting the immune response from T-helper-2 to T-helper-1. Clinical outcomes indicate lower mortality rates in PIA-immunized patients compared to unimmunized ones, especially in cases of vascular pythiosis. However, PIA immunotherapy may not provide a significant advantage in cases of ocular pythiosis.
