Vitamin C, Thiamine, and Hydrocortisone in the Treatment of Sepsis: A Meta-Analysis and Trial Sequential Analysis of Randomized Controlled Trials

Yazan Zayed et al.

Summary: There is conflicting evidence regarding the benefit of vitamin C, thiamine, and hydrocortisone as an adjunctive therapy for sepsis with or without septic shock. The treatment did not show a significant reduction in mortality, incidence of AKI, hospital and ICU length of stay, or ICU free days on day 28, but did show a significant reduction of SOFA score on day 3 post-randomization. Further studies with larger patient cohorts are needed to determine the efficacy of this treatment.

Summary: HAT therapy did not show significant benefits in terms of hospital mortality, ICU mortality, and length of stay for patients with sepsis, but it did reduce the duration of vasopressor use. Further research is needed to fully understand its potential benefits.

Micronutrients early in critical illness, selective or generous, enteral or intravenous?

Mette M. Berger et al.

Summary: Although micronutrients are essential in nutrition, their administration and monitoring are challenging in critically ill patients. Recent clinical trials testing high doses of single micronutrients or combinations have not shown clinical benefits in sepsis. Future trials need to consider the interconnected activity of micronutrients and choose more specific endpoints.

Thiamine combined with vitamin C in sepsis or septic shock: a systematic review and meta-analysis

Zengzheng Ge et al.

Summary: Despite the widespread enthusiasm for thiamine combined with vitamin C in patients with sepsis or septic shock, there was no statistical difference in in-hospital mortality. However, improvements were seen in SOFA scores and duration of vasopressor use within 72 hours of enrollment.

Summary: A trial on the effects of a combination treatment of vitamin C, thiamine, and hydrocortisone on ventilator- and vasopressor-free days in patients with sepsis did not show a significant increase within 30 days. However, the trial was terminated early and may have been underpowered to detect a clinically important difference.

Neuroprotective Effects of Thiamine and Precursors with Higher Bioavailability: Focus on Benfotiamine and Dibenzoylthiamine

Margaux Sambon et al.

Summary: Thiamine thioesters, such as benfotiamine (BFT) and dibenzoylthiamine (DBT), have shown potential benefits in treating neurodegenerative diseases with antioxidant and anti-inflammatory properties. Recent studies suggest that DBT may be even more efficient than BFT in terms of anti-inflammatory potency. These thiamine thioesters have pleiotropic properties and may lead to promising advancements in neurodegenerative, neurodevelopmental, and psychiatric conditions.

The relation between acute changes in the systemic inflammatory response and circulating thiamine and magnesium concentrations after elective knee arthroplasty

Donogh Maguire et al.

Summary: This study aimed to investigate the relationship between acute changes in the systemic inflammatory response (SIR) and thiamine and magnesium dependent enzyme activity in patients undergoing elective knee arthroplasty. The findings showed that pre-operatively TDP and magnesium concentrations were normal, but post-operatively TDP concentrations increased while magnesium concentrations decreased. Thiamine and magnesium could be potentially modifiable therapeutic targets to modulate the host inflammatory response.
