Absolute Quantitation of Cardiac 99mTc-Pyrophosphate Using Cadmium-Zinc-Telluride-Based SPECT/CT

Summary: Absolute quantitation of Tc-99m-pyrophosphate provides accurate assessment and diagnosis of cardiac amyloidosis. The quantitative metrics show moderate positive correlations with LVMI (left ventricular mass index) and strong positive correlations with ECV (extracellular volume). Excellent intraobserver repeatability of the quantitative metrics was observed.

The Working Group on Myocardial and Pericardial Disease has proposed definitions and diagnostic algorithms for cardiac amyloidosis.

Changes in the perceived epidemiology of amyloidosis: 20 year-experience from a Tertiary Referral Centre in Tuscany

Summary: A 20-year epidemiological study of amyloidosis patients at Careggi University Hospital in Florence, Italy found significant changes in the distribution of different types of amyloidosis over time. Wild type transthyretin amyloidosis (wtATTR) emerged as the most common etiology at the end of the observation period, contrasting with the previous absence of recognition before 2009. Serum amyloid A amyloidosis (AA) remained a residual diagnosis throughout the entire examined period.

False positive technetium-99m pyrophosphate scintigraphy in a patient with cardiac amyloidosis light chain Case report

False-positive bone scintigraphy denoting transthyretin amyloid in elderly hypertrophic cardiomyopathy

Summary: A case study of an 85-year-old man with positive bone scintigraphy but diagnosed with sarcomeric hypertrophic cardiomyopathy through endomyocardial biopsy. Genetic study did not show mutations in TTR gene but suggested involvement of sarcomeric genes. In hypertrophic cardiomyopathy, factors such as cell necrosis related to oxygen mismatch or small vessels disease could lead to false-positive bone scintigraphy for transthyretin amyloidosis, highlighting the importance of endomyocardial biopsy for accurate diagnosis.
