Summary: Research on the effects of dynamic hydrostatic pressure on the intervertebral disc (IVD) found that low-loading pressure was beneficial for cell survival and ECM homeostasis, while high magnitude pressure aggravated ECM breakdown. The fibrocartilaginous inner annulus fibrosus (AF) showed greater tolerance to pressure, and differential expression of mechanosensing factors affected cell fate under pressure. This study provides insights into the role of hydrostatic pressure in regulating cellular fate and matrix metabolism of the IVD.

Candidate genes such as COL1A1, COL2A1, COL9A2, COL9A3, COL11A1, and COL11A2 were found to play a significant role in the genetic basis of collagen degeneration in IVDs. Understanding the etiology of impaired collagen formation can lead to pathogenetically-based treatment for more effective results.

Targeted TRPV4 inhibition or knockdown may constitute promising therapeutic approaches for treating IVD pathologies caused by aberrant mechanical stress.
