Clinical Application of Trans-Arterial Radioembolization in Hepatic Malignancies in Europe: First Results from the Prospective Multicentre Observational Study CIRSE Registry for SIR-Spheres Therapy (CIRT)

Thomas Helmberger et al.

Summary: The CIRT study analyzed 1027 patients and found that a majority of treatment intentions were palliative. Various malignant tumors had different median overall survival rates, and significant prognostic factors for survival included the presence of ascites, cirrhosis, extra-hepatic disease, patient performance status (ECOG), number of chemotherapy lines prior to TARE, and tumor burden.

Proton beam radiotherapy vs. radiofrequency ablation for recurrent hepatocellular carcinoma: A randomized phase III trial

Tae Hyun Kim et al.

Summary: The study compared proton beam radiotherapy (PBT) and radiofrequency ablation (RFA) outcomes in patients with recurrent/residual HCC, demonstrating non-inferiority of PBT in terms of 2-year local progression-free survival (LPFS). The most common adverse events with PBT were radiation pneumonitis and decreased leukocyte counts, while RFA resulted in increased alanine aminotransferase levels and abdominal pain. PBT was found to be a tolerable and safe treatment option for patients with small recurrent hepatocellular carcinoma.

Variceal bleeding is aggravated by portal venous invasion of hepatocellular carcinoma: a matched nested case-control study

Summary: In patients with portal vein tumor thrombosis (PVTT), there is an increased risk of high-risk esophageal varices and variceal bleeding. Endoscopic prevention may be considered for high-risk variceal carriers with PVTT, especially those undergoing sorafenib treatment for HCC.

Personalised versus standard dosimetry approach of selective internal radiation therapy in patients with locally advanced hepatocellular carcinoma (DOSISPHERE-01): a randomised, multicentre, open-label phase 2 trial

Etienne Garin et al.

Summary: This study compared the efficacy of personalized versus standard dosimetry in selective internal radiation therapy for advanced hepatocellular carcinoma, showing that personalized dosimetry significantly improved the objective response rate. The results suggest that personalized dosimetry may enhance outcomes in clinical practice and should be considered for future trials.

Locoregional therapies in the era of molecular and immune treatments for hepatocellular carcinoma

Josep M. Llovet et al.

Summary: Locoregional therapies, including local ablation and transarterial chemoembolization, play a crucial role in the management of hepatocellular carcinoma. Recent studies have shown potential benefits of combining these local therapies with systemic treatments, such as molecular therapies, to enhance therapeutic effects.

Standardization of conventional chemoembolization for hepatocellular carcinoma

Summary: The study evaluated the chemical and physical characteristics of emulsions for cTACE and assessed intra-and inter-operator variabilities in the preparation processes. The results showed that droplet diameter changed with the number of pumping exchanges and over time, but returned to initial sizes after remixing. The study suggests that any interventional radiologist can prepare these emulsions, regardless of experience, and these data may provide guidelines for standardizing cTACE procedures.

The impact of portal vein tumor thrombosis on survival in patients with hepatocellular carcinoma treated with different therapies: A cohort study

Aline Maehringer-Kunz et al.

Summary: Portal vein tumor thrombosis (PVTT) is a common complication of hepatocellular carcinoma (HCC) classified as advanced stage disease. This study analyzed treatment-naive HCC patients with PVTT to determine the impact of different treatments based on the extent of PVTT. Resection may be a more effective treatment option for patients with segmental PVTT (Vp1) compared to systemic therapy.
