Fertility preservation in women for medical and social reasons: Oocytes vs ovarian tissue

Marie-Madeleine Dolmans et al.

Summary: Approximately 10% of cancers occur in women under the age of 45, with chemotherapy, radiotherapy, and bone marrow transplantation curing more than 90% of cases but potentially leading to premature ovarian insufficiency. Fertility preservation may also be necessary for benign diseases or social reasons. Oocyte vitrification and ovarian tissue cryopreservation are two main strategies for preserving fertility in female cancer patients.

Duration of estrogen exposure during reproductive years, age at menarche and age at menopause, and risk of cardiovascular disease events, all-cause and cardiovascular mortality: a systematic review and meta-analysis

S. R. Mishra et al.

Summary: This study evaluated estrogen exposure measurement and the mutual effect of age at menarche and age at menopause on the risk of cardiovascular disease events. Reproductive lifespan was the most commonly used method for estrogen exposure measurement. The findings suggest that a shorter reproductive lifespan is associated with a higher risk of cardiovascular disease events, particularly stroke.

Effects of letrozole or tamoxifen coadministered with a standard stimulation protocol on fertility preservation among breast cancer patients

Yael Shulman et al.

Summary: A retrospective study of 118 breast cancer patients undergoing fertility preservation treatment found that letrozole or tamoxifen did not significantly affect the number of retrieved oocytes, fertilization rate, or duration of stimulation, but did impact top-quality embryo rate. Further studies are needed to confirm these findings and explore the potential negative effects of letrozole or tamoxifen on fertility preservation outcomes.

Oocyte vitrification for fertility preservation for both medical and nonmedical reasons

Ana Cobo et al.

Summary: The oocyte vitrification technique has been widely used in the field of fertility preservation, benefiting women at risk of losing ovarian function. Age is a critical factor affecting reproductive prognosis, and elective fertility preservation patients should make decisions before the age of 35. Diseases and surgeries such as cystectomy before oocyte retrieval can impact reproductive outcomes.

Transplantation of cryopreserved ovarian tissue in a series of 285 women: a review of five leading European centers

Marie-Madeleine Dolmans et al.

Summary: The feasibility of freezing and thawing ovarian tissue is well-documented, but ovarian tissue transplantation is progressing at a slower pace with limited clinical experience. Results from five European centers show a low risk of relapse with ovarian tissue reimplantation and successful recovery of endocrine function in most patients, although in vitro fertilization efficacy needs improvement. Chemotherapy before ovarian tissue cryopreservation does not significantly affect success rates. Further research is needed to optimize the approach for ovarian tissue transplantation.

The program enrolls patients between 0-17 years old diagnosed with fertility-threatening conditions or undergoing gonadotoxic treatment. Results show that the FP procedures are safe and well-received by patients and parents, enhancing the quality of care for pediatric patients facing fertility-threatening diagnoses or treatments.

Summary: The study evaluated in vitro maturation (IVM) outcomes in 133 chemotherapy naive patients aged 1-35 years. The results showed a gradual increase in IVM rates from 1 to 24 years, with a peak in the 18-24 years group and a decrease in the 25-35 years group. Significantly lower success rates were observed in patients under 6 years old and over 30 years old compared to the 18-24 years group.

Summary: The simultaneous stimulation of follicular phase and luteal phase in the same menstrual cycle, known as double stimulation/dual stimulation, significantly increases the number of oocytes retrieved, particularly beneficial for poor ovarian responders. However, limitations such as freeze-all requirement and lack of cost-effectiveness data still exist for this novel approach.
