Anti-CHI3L1, anti-DLL-4, and anti-SFTPD IgM autoantibodies correlated with worse outcomes, and detection of C3d on cholangioctyes and antigen-specific autoreactive T cells suggests that autoimmunity plays a role in ongoing bile duct injury and disease progression.

Life long follow up and management strategies of patients living with native livers after Kasai portoenterostomy

Summary: The study followed 231 patients who underwent open KPE and found that over 66% of patients were jaundice-free at 1 year post-surgery, with stable native liver survival rates of 70.7% and 61.5% at 10 and 20 years respectively. Identification of a wide spectrum of ciliary gene mutations in nonsyndromic biliary atresia patients implicates ciliary dysfunction as a novel disease mechanism

Functional analysis showed absence of cilia in BA livers with KIF3B and TTC17 mutations, and knockdown of PCNT, KIF3B and TTC17 resulted in reduced number of cilia in patients. Additionally, engineered zebrafish knockouts of KIF3B, PCNT and TTC17 displayed reduced biliary flow.

Selective deficiency of Cdc42 in fetal mouse cholangiocytes led to histological changes resembling those in human BA livers, indicating the crucial role of Cdc42 in BA development.
