Vascular histopathology and connective tissue ultrastructure in spontaneous coronary artery dissection: pathophysiological and clinical implications

Summary: Spontaneous coronary artery dissection (SCAD) is a cause of acute coronary syndromes and, in rare cases, sudden cardiac death (SCD). Connective tissue abnormalities, coronary inflammation, increased coronary vasa vasorum (VV) density, and coronary fibromuscular dysplasia have been implicated in the pathophysiology of SCAD, but were not systematically assessed before. The study found that peri-coronary inflammation in SCAD is distinct from vasculitides, and coronary fibromuscular dysplasia or increased VV density does not appear to be a significant pathophysiological factor. Dermal connective tissue changes are also not common in SCAD survivors.

The European/International Fibromuscular Dysplasia Registry and Initiative (FEIRI)-clinical phenotypes and their predictors based on a cohort of 1000 patients

Summary: The study aimed to compare characteristics of patients with renal infarction related to nontrauma renal artery dissection with or without underlying vascular disease, and report long-term renal and vascular outcomes. The results showed a similar frequency of new-onset RAD in patients with aneurysmal multisite arterial disease and fibromuscular dysplasia, but a higher rate of de novo extrarenal dissection in the former group.

National French registry of spontaneous coronary artery dissections: prevalence of fibromuscular dysplasia and genetic analyses

Summary: This study analyzed data from a national French registry of SCAD cases, revealing a mean age of 51.5 years among SCAD patients, with majority being women and most cases having less than two cardiovascular risk factors. After one year of follow-up, 79.1% of patients were treated conservatively, with a 12.3% MACE rate and no mortality cases, while the recurrence rate of SCAD was 3.3%. Fibromuscular dysplasia (FMD) was found in 45.0% of cases, and a genetic association with the PHACTR1 locus was confirmed.
