Hemorrhagic risk after intravenous thrombolysis for ischemic stroke in patients with cerebral microbleeds and white matter disease

Maria Luisa Capuana et al.

Summary: The study aimed to evaluate the association between cerebral microbleeds (CMBs) and white matter disease (WMD) with intracerebral hemorrhage (ICH) after IV thrombolysis (IVT) with rt-PA. Results suggest that excluding eligible candidates for IV rt-PA treatment solely based on the presence of CMBs is not justified. Clinical decisions should be made on a case-by-case basis and more data is needed to assess the benefit-risk profile of rt-PA in patients with numerous microbleeds.

Efficacy and Safety of Intravenous rtPA in Ischemic Strokes Due to Small-Vessel Occlusion: Systematic Review and Meta-Analysis

Bartosz Karaszewski et al.

Summary: Intravenous recombinant tissue plasminogen activator (iv-rtPA) has been used to treat ischemic stroke for 25 years, but there is limited evidence on its efficacy and safety in strokes caused by cerebral small vessel disease (SVD). While results suggest some benefit and safety in this specific subgroup, further research is needed to make definitive recommendations.

Development of imaging-based risk scores for prediction of intracranial haemorrhage and ischaemic stroke in patients taking antithrombotic therapy after ischaemic stroke or transient ischaemic attack: a pooled analysis of individual patient data from cohort studies

Jonathan G. Best et al.

Summary: This study developed new risk scores incorporating clinical variables and cerebral microbleeds for predicting the long-term risks of intracranial haemorrhage and ischaemic stroke in patients prescribed antithrombotic therapy for secondary stroke prevention. The risk models showed good predictive value and calibration, providing potential clinical utility. External validation of the models is recommended.
