Summary: This study retrospectively collected data on 57 patients with activated phosphoinositide 3-kinase delta syndrome (APDS) who underwent hematopoietic cell transplantation (HCT) and evaluated the outcomes. The study found that graft failure, graft instability, and poor graft function requiring unplanned donor cell infusion were major barriers to successful HCT. The use of mammalian target of rapamycin inhibitor (mTORi) post-transplantation may confer an advantage to residual host cells, promoting graft instability.
