Allorecognition and the spectrum of kidney transplant rejection

Summary: The detection of mismatched human leukocyte antigens by adaptive immune cells is the main cause of transplant rejection. However, recent observations have questioned the traditional dichotomy between T-cell mediated rejection and antibody-mediated rejection, leading to a need for a more integrated view of kidney transplant rejection. This review highlights the gaps in the current classification of rejection, provides an overview of the expanding insights into the mechanisms of allorecognition, and discusses how these insights could improve our understanding and clinical approach to kidney transplant rejection.

Risk factors, histopathological features, and graft outcome of transplant glomerulopathy in the absence of donor-specific HLA antibodies

Summary: Transplant glomerulopathy can occur independently of HLA-DSA, with less inflammation and better graft survival compared to cases with HLA-DSA. The presence of HLA-DSA was identified as a risk factor for glomerulopathy development, and HLA-DSA negative cases had a lower risk of graft failure compared to HLA-DSA positive cases. Landmark analysis showed that HLA-DSA negative patients with glomerulopathy still had a significant increased risk of graft failure, indicating the importance of monitoring and managing transplant glomerulopathy even in the absence of HLA-DSA.

Summary: Studies have shown conflicting results regarding the prognosis of HLA-DSA-negative hAMR cases, with some indicating better outcomes and others demonstrating poor survival. Possible pathophysiological mechanisms for HLA-DSA-negative hAMR include undetected HLA-DSA involvement, non-HLA-DSA involvement, and antibody-independent NK cell activation. More research is needed on measuring non-HLA antibodies, recipient/donor NK cell genotyping, and exploring antibody reduction therapies for this subset of patients.
