Mobile Plasmid Mediated Transition From Colistin-Sensitive to Resistant Phenotype in Klebsiella pneumoniae

Summary: This study identified a clinical CRKP strain capable of transitioning to a colistin-resistant phenotype by inserting the ISKpn72 element, which could also confer colistin resistance to sensitive K. pneumoniae strains. Furthermore, it was found that the ISKpn72-containing plasmid could be transferred through conjugation, indicating potential challenges for colistin-resistance detection and CRKP treatment.

The face of hypervirulent Klebsiella pneumoniae isolated from clinical samples of two Iranian teaching hospitals

Summary: Hypervirulent Klebsiella pneumoniae (hvKp) is a global concern pathogen, with a high percentage of multidrug-resistant hvKp (76.6%) and high resistance to imipenem (67%) indicating a serious issue that needs attention in the clinical setting. The majority of hvKp isolates belonged to capsular serotype K20 and ompK36 group C, which is related to clonal group (CG) 23 (e.g. ST23).

In vivo Emergence of Colistin Resistance in Carbapenem-Resistant Klebsiella pneumoniae Mediated by Premature Termination of the mgrB Gene Regulator

Summary: This study isolated a pair of carbapenem-resistant Klebsiella pneumoniae isolates from the same patient before and after colistin treatment, named KP1-1 and KP1-2. The Gln30* alteration in KP1-2 was found to be responsible for colistin resistance, and complementation with wild-type mgrB gene restored colistin susceptibility. Overexpression of phoP, phoQ, and pmrD genes in KP1-2 compared to KP1-1 confirmed the role of MgrB alterations in increased expression levels of these genes, providing direct in vivo evidence for colistin resistance in K. pneumoniae clinical strains.
