Mixed Neuroendocrine Carcinoma and Hepatocellular Carcinoma: A Case Report and Literature Review

Summary: We report an extremely rare case of a mixed neuroendocrine-non-neuroendocrine neoplasm in the liver, which was preoperatively misdiagnosed as hepatocellular carcinoma. The patient was successfully treated with platinum-based chemotherapy and showed no disease recurrence at 6-month follow-up. The case highlights the importance of proper diagnosis and treatment in managing this rare disease.

Comprehensive genetic features of gastric mixed adenoneuroendocrine carcinomas and pure neuroendocrine carcinomas

Jiwon Koh et al.

Summary: In this study, targeted DNA sequencing was conducted on tumor samples from 21 patients to investigate the pathogenesis of gastric mixed adenoneuroendocrine carcinoma (MANEC) and pure neuroendocrine carcinoma (NEC). Mutational profiles of MANECs/NECs were compared with other tumors, revealing unique mutational profiles and underlying protein networks for gastric MANEC and pure NEC. The research suggests a possible clonal origin of ADC and NEC components of MANEC, with differentially altered genes associated with specific signaling pathways.

Summary: ENEC is a rare tumor with high malignant potential and poor prognosis. The most common treatments include surgery, chemoradiotherapy, chemotherapy, and endoscopic submucosal dissection. Accurate pretreatment histological diagnosis is essential for determining the most appropriate treatment strategy. Surgery may have limited survival benefits for advanced stages of ENEC.

Bcl-10, trypsin and synaptophysin helps recognize acinar cell and mixed acinar neuroendocrine cell carcinoma of the pancreas on both preoperative cytological samples and needle biopsy specimens

Erminia Manfrin et al.

Summary: This study evaluated a diagnostic panel based on a limited number of ICC markers for typing preoperatively ACC of the pancreas. BCL10 was found to be a determinant marker for the diagnosis of acinar cell carcinoma and mixed acinar neuroendocrine cell carcinoma, with 100% sensitivity and specificity for ACC. Trypsin and synaptophysin were also helpful in correctly identifying ACC cases and those with mixed neuroendocrine components, respectively.

Summary: This study retrospectively analyzed the clinical data of 46 patients with gastroenteropancreatic MiNEN and found that Ki-67 index, tumor composition, lymph node involvement, distant metastasis, and clinical stage are important factors affecting patient prognosis. The study also showed that patients with NEC had a shorter overall survival compared to those with MiNEN, while the survival did not significantly differ from those with poorly differentiated adenocarcinoma and MiNEN.

Summary: This study utilized digital image analysis (DIA) to evaluate SSTR2 immunoreactivity in pancreatic neuroendocrine tumors (PanNETs) and found that DIA provided reproducible results. PanNETs with HER2 scores > 2 showed the highest concordance with strong positive cell percentages obtained by DIA when Minimum Membrane Completeness was set at 80%. Digital image analysis proved to be a reliable method for assessing SSTR2 immunoreactivity and provided important insights for the potential use of SSAs.

Summary: MiNEN is a newly defined entity consisting of neuroendocrine and non-neuroendocrine tumor components.
