Summary: Dynamic HPA testing revealed significant differences in plasma steroid levels between patients with APA and BPA, with APA patients showing higher concentrations of specific steroids both at baseline and in response to ACTH stimulation. The discriminatory power of steroid measurements taken 15 minutes post-ACTH stimulation was particularly high, suggesting it could potentially replace adrenal vein sampling in some PA patients.

Summary: The study identified gain-of-function somatic mutations in GNA11 or GNAQ in CTNNB1-mutant aldosterone-producing adenomas. These mutations were often found in patients during puberty, pregnancy, or menopause, with elevated LHCGR expression. While most aldosterone-producing adenomas have gain-of-function somatic mutations, the presence of these mutations in aldosterone-producing cell clusters of normal adrenal glands indicates a necessity for codriver mutations in APAs.

Re-evaluating absent clinical success after adrenalectomy in unilateral primary aldosteronism

Summary: This study assessed the clinical outcomes of 104 patients with unilateral primary aldosteronism after adrenalectomy, revealing that some patients were classified as having absent clinical success post-surgery. However, after a follow-up of 5.6 years, 12 patients showed partial or complete clinical success upon reassessment. Despite the variability in clinical outcomes, unilateral adrenalectomy remains the recommended treatment for patients with lateralized primary aldosteronism.
