Summary: The lung is the primary organ affected by severe coronavirus disease 2019 (COVID-19) caused by the novel coronavirus SARS-CoV-2, with lung damage being the leading cause of death in most patients. Pathological features of fatal COVID-19 include early changes like edema, epithelial damage, and capillaritis/endothelialitis, progressing to exudative diffuse alveolar damage (DAD) in patients with respiratory insufficiency. These features are not specific to COVID-19 and can be found in other disorders, while clinically, early severe COVID-19 stages are characterized by high viral load, lymphopenia, pro-inflammatory cytokine secretion, and hypercoagulability.
