Neonatal platelet physiology and implications for transfusion

Francisca Ferrer-Marin et al.

Summary: The neonatal hemostatic system differs from that of adults, with developmental differences in platelet function and primary hemostasis. Factors in neonatal blood such as high von Willebrand factor levels increase platelet-vessel wall interaction, leading to shorter bleeding times in healthy full-term infants. However, prophylactic platelet transfusions do not decrease bleeding in preterm neonates and may increase neonatal morbidity and mortality.

Heightened activation of embryonic megakaryocytes causes aneurysms in the developing brain of mice lacking podoplanin

Christopher Hoover et al.

Summary: In mice, the ligand PDPN for CLEC-2 is temporarily expressed in neural tissue during midgestation and its loss leads to aneurysms and spontaneous hemorrhage, with enhanced granule release and localization of eMks/platelets in the lower diencephalon before hemorrhage. PDPN counteracted collagen-1-induced secretion of angiopoietin-1, leading to enhanced TIE-2 activation in aneurysm-like sprouts of PDPN-deficient embryos, and blocking platelet activation prevented vascular defects in the PDPN-deficient embryos.

Impacts of ABO-incompatible platelet transfusions on platelet recovery and outcomes after intracerebral hemorrhage

Jessica Magid-Bernstein et al.

Summary: Acute platelet transfusion after intracerebral hemorrhage that is ABO-incompatible may lead to poorer platelet recovery and neurological outcomes compared to ABO-compatible transfusions. Further research into the underlying mechanisms is needed to better understand these observations.

Implementation of a neonatal platelet transfusion guideline to reduce non-indicated transfusions using a quality improvement framework

Patricia E. Davenport et al.

Summary: Implementation of a restrictive neonatal platelet transfusion guideline significantly reduced potentially harmful platelet transfusions in our NICU without a change in major bleeding. Rates of non-indicated platelet transfusions decreased, while rates of major bleeding remained stable.

Benefits of lower neonatal platelet transfusion thresholds

Rida Hasan et al.

Summary: The degree of thrombocytopenia does not predict bleeding risk in neonates, yet platelet transfusions are often given prophylactically to non-bleeding premature infants. Recent data suggest that a lower platelet transfusion threshold of 25 x 10(9)/L in non-bleeding premature neonates may be safer, as higher thresholds could increase the risk of death and bleeding. The mechanism behind the increased adverse events with higher platelet transfusion threshold is still unknown, but potential factors include disruption of hemostatic balance and a pro-inflammatory effect.
