Summary: Patients with multiple sclerosis who have immunoglobulin gamma oligoclonal bands in the cerebrospinal fluid have different genetic backgrounds and brain MRI features compared to those who do not. The presence of these bands is associated with reaching disability milestones earlier and at a younger age, as well as an increased risk of converting to secondary progressive MS. These findings suggest a stronger disease-modifying effect of the bands in the early inflammatory stages of MS.
