The Glymphatic System: A Review of the Challenges in Visualizing its Structure and Function with MR Imaging

Summary: The central nervous system was previously thought to lack lymphatic vessels, but the recently hypothesized glymphatic system has challenged this belief. This system clears waste products from the brain and regulates immunity, and its impairment may contribute to neurodegenerative diseases. In vivo MR imaging studies have been used to visualize and understand the function and structure of the glymphatic system and meningeal lymphatics. This review summarizes these studies and discusses their significance, limitations, and future directions.

Noninvasive Characterization of Human Glymphatics and Meningeal Lymphatics in an in vivo Model of Blood-Brain Barrier Leakage

Summary: This study successfully demonstrated human glymphatics and meningeal lymphatics noninvasively using high-resolution 3D isotropic CE-T2-FLAIR imaging. Para-arterial, paravenous, and parasinus meningeal contrast enrichment were clearly depicted in patients with reversible cerebral vasoconstriction syndrome (RCVS) and cluster headache, suggesting universal anatomical structures rather than phase- or condition-specific. The technique may serve as a basis for further studies to delineate clinical relevance of glymphatic clearance.

Utility of Contrast-Enhanced T2 FLAIR for Imaging Brain Metastases Using a Half-dose High-Relaxivity Contrast Agent

Summary: This study investigated the clinical value of contrast-enhanced T2 FLAIR using half-dose gadobenate dimeglumine in imaging brain metastases, finding that the signal intensity of T2 FLAIR was maximal at one-eighth to one-half of the contrast concentration needed for T1-weighted imaging. Contrast-enhanced T2 FLAIR significantly increased contrast ratios, aiding in the classification and diagnosis of brain metastases.
