Summary: This study provides practical principles and examples to help GRADE users make optimal choices regarding their ratings of certainty of evidence using a minimally or partially contextualized approach. Systematic review and health technology assessment authors need to clarify their target of certainty rating, which depends on contextualization, thresholds, and estimate location. GRADE offers guidance for specifying the target of certainty of evidence rating.
