Posture Dependent Hypoxia Following Lobectomy: The Achilles Tendon of the Lung Surgeon?

Athanassios Krassas et al.

Summary: Platypnoea-Orthodeoxia Syndrome is a rare condition that causes dyspnea and arterial desaturation after lung resection, primarily due to intracardiac anomalies resulting in a right-to-left shunt.

Atrial septostomy in patients supported with venoarterial extracorporeal membrane oxygenation: Analysis of the IMPACT registry data

Shriprasad R. Deshpande et al.

Summary: This study utilized the IMPACT registry to investigate the utilization, safety, and outcomes of atrial septostomy in children supported on venoarterial ECMO. Results showed significant procedural complications but no procedural mortality, with an overall hospital mortality rate of 46%. Factors such as race, chronic lung disease, and procedural urgency were associated with early post-procedural mortality. Further studies on these factors are warranted.

Fetal echocardiographic predictors of biventricular circulation in hypoplastic left heart complex

K. Haberer et al.

Summary: The study found that features such as right-to-left foramen ovale flow and higher mitral valve annulus Z-scores are important predictors of biventricular circulation in fetal HLHC patients. Additionally, certain characteristics such as FO flow direction and specific Z-scores can effectively predict the failure of BV circulation.

Left heart growth and biventricular repair after hybrid palliation

Vladimir Sojak et al.

Summary: Hybrid palliation can promote left heart growth in some patients with left ventricle hypoplasia, leading to more patients eventually achieving biventricular circulation than initially anticipated. Additional interventions and operations may be required for these patients. Despite ventricular rehabilitation, patients with borderline left ventricles may develop restrictive physiology.

Super Glenn for staged biventricular repair: impact on left ventricular growth?

Supreet P. Marathe et al.

Summary: The Super Glenn procedure was found to consistently increase left ventricular dimensions in patients, potentially aiding in achieving successful biventricular circulation in those with borderline left ventricle. However, the superiority/non-inferiority of this approach over the conventional Fontan pathway remains unclear.

Modalities of Left Ventricle Decompression during VA-ECMO Therapy

Juan Pablo Ricarte Bratti et al.

Summary: VA-ECMO is used for circulatory and respiratory support in patients with severe cardiogenic shock or refractory cardiac arrest, but it may have detrimental effects on myocardial recovery, leading to left ventricular distention. Therefore, recognizing, preventing, and treating LV distention is crucial in the management of these patients.

Platypnea-Orthodeoxia Syndrome: Rare or Under-Diagnosed Syndrome? 3 Case Reports and a Literature Review

Isaac Yepes et al.

Summary: Platypnea-Orthodeoxia Syndrome (POS) is characterized by dyspnea and hypoxemia in the upright position, which improves when lying down, typically caused by congenital interatrial communications leading to right to left shunting. Echocardiographic imaging can confirm the shunting, and transcatheter closure of the communication is highly successful in relieving symptoms and resolving hypoxemia in most patients.
