In vitro Hemocompatibility Evaluation of the HeartWare Ventricular Assist Device Under Systemic, Pediatric and Pulmonary Support Conditions

Summary: The development of adult use right ventricular assist devices (RVADs) and pediatric left ventricular assist devices (pediatric LVADs) has significantly lagged behind compared to adult use left ventricular assist devices (LVADs). The off-label use of the HeartWare ventricular assist device (HVAD) for adult pulmonary and pediatric systemic support has shown significant differences in hemocompatibility profiles compared to its intended usage state. In vitro hemocompatibility testing of HVAD in different support conditions revealed substantial variations in hemolysis and von Willebrand factor multimer degradation, indicating the importance of considering usage mode in clinical applications and future device development.

Computed tomography angiography as an adjunct to computational fluid dynamics for prediction of oxygenator thrombus formation

Summary: In this study, the use of computational fluid dynamics and computed tomography angiography for analyzing thrombus formation in Quadrox D oxygenators was investigated. The results showed a well-defined pattern of clot burden in the oxygenators, consistent with the predicted values from computed tomography angiography. The study suggests that computed tomography angiography-based flow analysis is a valuable adjunct to computational fluid dynamics in understanding oxygenator thrombus formation.

Summary: The STS-Intermacs 2020 Annual Report analyzed data from 25,551 patients who underwent LVAD implantation between 2010 and 2019, revealing an increase in African American patients and a higher proportion bridged to durable LVAD with temporary mechanical support devices. Hemolysis at low blood flow rates: in-vitro and in-silico evaluation of a centrifugal blood pump

Summary: This study highlights the underappreciated hemolysis in centrifugal pumps within the low-flow range, and stresses the urgent need for a specific pump optimized for low-flow treatment. Due to inherent problems of available centrifugal pumps in the low-flow range, caution should be taken when using current centrifugal pumps, and alternative pumping principles may be discussed in the future.

Extracorporeal Membrane Oxygenation-Induced Hemolysis: An In Vitro Study to Appraise Causative Factors

Summary: This study suggests that the choice of anticoagulant and blood donor gender play a critical role in hemolysis testing during ECMO, and should be considered to improve testing reliability. Hemolysis in heparinized blood increased significantly over time, while hemolysis in citrated blood remained unchanged. The difference in hemolysis between different flow rates and genders of blood donors was also observed.

Summary: Mechanically assisted circulation (MAC) supports blood circulation temporarily or long-term in patients undergoing cardiac surgery or on ventricular assistance with a ventricular assist device (VAD) or on extracorporeal membrane oxygenation (ECMO). However, the non-physiological shear stress (NPSS) during MAC can cause blood damage and functional alterations, leading to perioperative bleeding and thrombotic events that impact morbidity and mortality in MCS patients. Understanding shear-induced hemostatic disorders is crucial for developing low-shear-stress devices and improving clinical outcomes.
