Perioperative Mortality After Cemented or Uncemented Hemiarthroplasty for Displaced Femoral Neck Fractures-A Systematic Review and Meta-analysis

Summary: This study found that compared to uncemented hemiarthroplasty, patients undergoing cemented hemiarthroplasty have a significantly higher mortality rate within the first 48 hours of surgery, but no difference at 7 days, 30 days, and one year. Surgeons should consider tailoring their stem choice based on the physiological status of the patient when planning hemiarthroplasty for femoral neck fractures and techniques to reduce the risk of bone cement implantation syndrome should be utilized.

Summary: The study found that among patients over the age of 70, those treated with arthroplasty for displaced FNF had a higher risk of mortality within 30 days post-surgery compared to those treated with IF, but no difference after 1 and 5 years; however, arthroplasty patients had lower risk of reoperation compared to IF patients.

Cemented versus uncemented hemiarthroplasty for the management of femoral neck fractures in the elderly: a meta-analysis and systematic review

Summary: Cemented hemiarthroplasty was found to have better outcomes in terms of reoperation rate, prosthesis-related complications, residual pain, and operation time compared to uncemented hemiarthroplasty. However, there were no significant differences between the two groups in terms of local and general complications, duration of hospital stay, hip function, and mortality. Further studies are needed to strengthen the evidence.

Treatment of femoral neck fractures: sliding hip screw or cannulated screws? A meta-analysis

Summary: This meta-analysis compared the clinical outcomes and complications of SHS and CCS in treating femoral neck fractures. It found that SHS had a higher rate of avascular necrosis, while CCS had a higher rate of implant removal. There were no significant statistical differences in other outcomes between the two groups.

Summary: Despite ample evidence supporting cemented femoral fixation as a preferred method for surgical treatment of displaced femoral neck fractures, cementless fixation is more commonly used in the United States. This study analyzed a large database comparing revision rates based on femoral fixation methods for hemiarthroplasty and THA, and found that cemented fixation is associated with lower revision rates for hemiarthroplasty but not for THA in the United States.

Osteosynthesis of non-displaced femoral neck fractures in the elderly population using the femoral neck system (FNS): short-term clinical and radiological outcomes

Summary: This study evaluated the short-term clinical and radiological outcomes after osteosynthesis of non-displaced FNF using TS, DHS, and FNS. The results showed that FNS is a valid alternative implant for FNF osteosynthesis with a shorter operative time compared to TS and DHS. Short-term outcomes of FNS are similar to TS and DHS, but further long-term studies are needed for confirmation.

Internal fixation implants for intracapsular hip fractures in older adults

Summary: This research evaluates the evidence for different types of internal fixation implants used in joint-preserving surgery for intracapsular hip fractures in older adults. The study found that there were no significant differences in early mortality, 12-month mortality, and unplanned return to theatre between the use of smooth pins and fixed angle plates for hip fractures. Similarly, there were no significant differences in functional status and health-related quality of life between screws and fixed angle plates. Further research is needed to increase the certainty of these outcomes.

Summary: Comparing two techniques for displaced intracapsular hip fractures, it was found that using cannulated cancellous screw (CCS) resulted in shorter operative time and reduced costs. Therefore, CCS should be preferred for the treatment of this type of fractures.
