Can we back off using antibiotics in the NICU?

Summary: Antibiotics are prescribed extensively and inconsistently in neonatal ICUs, with usage not correlating with culture positive sepsis rates. Overuse of antibiotics in neonates is associated with short and long-term adverse effects, including the burden of multi-drug resistant organisms. Variation in antibiotic prescribing practices among neonatologists highlights the need for antibiotic stewardship to standardize use and improve outcomes in this population. Various strategies have been proposed to identify neonatal sepsis and reduce antibiotic utilization, but all have limitations and may involve unnecessary treatment of non-septic infants. A watch and wait algorithm is suggested as an alternative method for initiating antibiotics in the NICU, relying on trained personnel for timely assessment.
