Gastric per-oral endoscopic myotomy (G-POEM) for refractory gastroparesis: results from an international prospective trial

Summary: Although G-POEM is considered a safe procedure, it only showed modest overall effectiveness in the treatment of refractory gastroparesis. High baseline GCSI Score, high baseline gastric retention, and early response to G-POEM were independent predictors of clinical success at 12 months. Summary: G-POEM is a minimally invasive and effective treatment for postsurgical gastroparesis, showing significant improvement in symptoms and gastric emptying. The severity of the condition did not affect the efficacy of G-POEM. Additionally, G-POEM also improved gastroesophageal reflux symptoms without any complications requiring re-hospitalization.

Food Residue During Esophagogastroduodenoscopy Is Commonly Encountered and Is Not Pathognomonic of Delayed Gastric Emptying

Summary: This study found that retained gastric food (RGF) was identified in 3% of esophagogastroduodenoscopies (EGD) performed on 85,116 patients with structural abnormalities. The prevalence of RGF was associated with factors such as type 1 diabetes, type 2 diabetes, gastroparesis, and amyloidosis. The positive predictive value (PPV) of RGF for delayed gastric emptying varied depending on the underlying risk factors, such as diabetes type and gastroparesis. Independent of delayed gastric emptying, the use of certain medications like acid suppressants, cardiovascular medications, and opioids were associated with RGF.

Gastric-emptying results were unreliable in capturing the pathophysiology of clinical symptoms in either condition, suggesting that FD and gastroparesis should be considered as part of the same spectrum of gastric neuromuscular disorders.
