Summary: Gastrectomy with lymphadenectomy is recommended for type 3 G-NETs based on lymph node metastasis risk. However, endoscopic resection for type 3 G-NETs <= 10 mm, confined to the mucosa or submucosa, with a grade of G1 showed excellent survival outcomes as an alternative treatment option despite the risk of LNM.

Summary: Although minimally invasive endoscopic resection for large upper gastrointestinal subepithelial tumors is limited, this study found a relatively high rate of complete resection and low rate of complications in a large case series. Tumor size was identified as an independent risk factor for incomplete resection and total complications. No local recurrence occurred during the median follow-up of 36 months.
