'Hot phase' clinical presentation in arrhythmogenic carci;omyopathy

Riccardo Bariani et al.

Summary: This study evaluated the clinical features of patients with arrhythmogenic cardiomyopathy presenting with chest pain and myocardial enzyme release in the setting of normal coronary arteries. It found that the 'hot phase' is an uncommon clinical presentation of AC, often occurring in pediatric patients and carriers of desmoplakin gene mutations. Tissue characterization, family history, and genetic testing are essential tools for differential diagnosis.

Transcriptomic Coupling of PKP2 With Inflammatory and Immune Pathways Endogenous to Adult Cardiac Myocytes

Marta Perez-Hernandez et al.

Summary: In cardiac myocytes, deficiency of PKP2 results in upregulation of a large number of transcripts, which are inversely correlated with PKP2 abundance in human transcriptomes and are associated with functional pathways related to inflammatory/immune responses.

Excess TGF-β1 Drives Cardiac Mesenchymal Stromal Cells to a Pro-Fibrotic Commitment in Arrhythmogenic Cardiomyopathy

Angela Serena Maione et al.

Summary: This study found that fibrotic markers are increased in cardiac biopsies of ACM patients compared to healthy controls, along with significantly higher levels of circulating TGF-beta1 in the ACM cohort. While this difference is not evident in isolated C-MSC, ACM C-MSC are more responsive to TGF-beta1 treatment, indicating their involvement in ACM fibrotic remodeling.

Summary: After a thorough review, only 8 genes have been definitively or moderately linked to ARVC, including PKP2, DSP, DSG2, DSC2, JUP, TMEM43, PLN, and DES. Pathogenic/likely pathogenic variants in these genes should be the main criteria for ARVC diagnosis, while variants in other genes require further investigation.
