Role of Imaging in Bronchoscopic Lung Volume Reduction Using Endobronchial Valve State of the Art Review

Chandra Dass et al.

Summary: The advancements in lung volume reduction surgery and bronchoscopic lung volume reduction procedures provide new hope for patients with COPD, especially those who are not suitable for surgery. The deployment of one-way endobronchial valves (EBVs) has shown to be effective in clinical utility, with imaging playing a key role in patient selection, target lobe identification, and post-procedural management.

miRNAomics analysis reveals the promoting effects of cigarette smoke extract-treated Beas-2B-derived exosomes on macrophage polarization

Zhen Chen et al.

Summary: Cigarette smoke-induced inflammation in the lungs, leading to conditions like COPD, is influenced by macrophage polarization and cross-talk between alveolar macrophages and lung epithelial cells via exosomes and miRNAs. This study identified differentially expressed exosomal miRNAs and their target genes involved in regulating macrophage polarization pathways. Furthermore, miR-21-3p and miR-27b-3p were found to play critical roles in promoting macrophage polarization in response to cigarette smoke exposure.

Chronic lung diseases: prospects for regeneration and repair

Peter J. Barnes et al.

Summary: COPD and IPF present significant unmet medical needs, and current treatment lags behind other chronic conditions. Early identification and personalized treatment, as well as understanding disease mechanisms, are crucial for disease reversal and lung regeneration. This review discusses advancements in pathology and molecular mechanisms to identify potential targets for disease degeneration reversal and tissue repair.

Inhibition of miR-494-3p alleviates oxidative stress-induced cell senescence and inflammation in the primary epithelial cells of COPD patients

Qinghua Zeng et al.

Summary: MiR-494-3p expression is upregulated by oxidative stress through the p38MAPK-c-myc signaling pathway, leading to direct binding to SIRT3 and reduction of its expression, resulting in increased cellular senescence and contributing to COPD progression.

Inhaled RNA Therapeutics for Obstructive Airway Diseases: Recent Advances and Future Prospects

You Xu et al.

Summary: Obstructive airway diseases, such as COPD and asthma, are major global health issues with insufficient treatment options. This review explores the challenges and potentials of current inhaled drugs as well as novel therapeutics like RNA treatments. The discussion covers quality attributes of inhaled medicines, advances in solid dosage forms for pulmonary administration, obstacles for clinical translation, delivery challenges for inhaled RNA therapeutics, and future prospects for RNA-based medicine in treating obstructive airway diseases.

Pulmonary transplantation of alpha-1 antitrypsin (AAT)-transgenic macrophages provides a source of functional human AAT in vivo

Ewa Janosz et al.

Summary: The study investigated the potential therapy of pulmonary transplantation of macrophages transgenic for AAT in treating AAT deficiency-associated lung pathology. The results demonstrated that in vitro generated AAT transgenic macrophages successfully engrafted in mice and showed efficient AAT expression and secretion, indicating the applicability of the vectors in human cells.

Summary: The expression of miR-181b-3p, miR-429, and miR-23c is increased in endothelial cells of COPD patients, with over-expression of miR-181b-3p associated with reduced sprouting. miR-429 is also overexpressed in lung cancer and exhibits a reduction in tubular formation.

Reversal of emphysema by restoration of pulmonary endothelial cells

Shu Hisata et al.

Summary: The study demonstrates the importance of endothelial cells in the pathogenesis of COPD/emphysema, showing that loss of endothelial cells can lead to alveolar destruction. Intravenous delivery of healthy lung endothelial cells rescued this destruction, suggesting regenerative methods or inhibiting the LRG1 pathway could be potential strategies for treating COPD/emphysema.
