Clinical manifestations and immunomodulatory treatment experiences in psychiatric patients with suspected autoimmune encephalitis: a case series of 91 patients from Germany

Dominique Endres et al.

Summary: Autoimmune encephalitis can manifest predominantly as psychiatric symptoms, especially in anti-NMDA receptor encephalitis. However, diagnosis in some patients is based on nonspecific findings and further research is needed.

High frequency of cerebrospinal fluid autoantibodies in COVID-19 patients with neurological symptoms

Christiana Franke et al.

Summary: The study on critically ill COVID-19 patients presenting with unexplained neurological symptoms found that all patients showed anti-neuronal autoantibodies in serum or CSF, which may be related to clinical symptoms, particularly hyperexcitability. Several underlying autoantigens and their potential molecular mimicry with SARS-CoV-2 still need further identification.

Summary: The study found that NMDAR antibodies were not common in FEP patients without anti-NMDAR encephalitis, and the warning signs and criteria for autoimmune psychosis had limited utility when neurological symptoms were absent or paraclinical tests were normal.

Limitations of a Commercial Assay as Diagnostic Test of Autoimmune Encephalitis

Raquel Ruiz-Garcia et al.

Summary: The detection of neuronal surface antibodies (NSAb) is crucial for the diagnosis of autoimmune encephalitis (AE), but commercial diagnostic kits based on indirect immunofluorescence on transfected cells (IIFA) may lead to false negative results, particularly in cases of anti-LG1, GABA(B)R, or AMPAR encephalitis. More comprehensive antibody studies are recommended when commercial IIFA testing yields negative results in suspected cases of these disorders.

Autoantibodies in neurological disease

Harald Pruess

Summary: The realization of the impact of autoantibodies on brain function has led to a paradigm shift in neurological diseases, providing new insights into central nervous system autoimmunity and driving clinical and experimental research. While specific autoantibodies have been identified, the search for more continues with promising results.
