Summary: The study aimed to evaluate the potential of a comprehensive machine learning risk score incorporating circulating biomarkers and CT measures in predicting long-term hard cardiac events in asymptomatic subjects. The ML risk score outperformed traditional CAC score and ASCVD risk score, with serum biomarkers providing incremental prognostic value. Novel biomarkers like MMP-9, pentraxin 3, PIGR, and GDF-15 played important roles in the ML algorithm and reflected pathways of inflammation, extracellular matrix remodeling, and fibrosis.
